Nurse Led Outreach Team
The Nurse Lead Outreach Team (NLOT) is a team of nurse practitioners and one registered nurse who work for Thunder Bay Regional Health Sciences Centre. This team works within long term care, assisted living, and retirement homes. The members of this team have experience in acute care nursing and caring for older adults.
What We Do
The nurse practitioners are available on scheduled days to assess residents with urgent medical concerns or illnesses, and to follow up on those recently discharged from the hospital. Our goal is to ensure timely, high quality care is provided within the comfort of a person’s home, while also ensuring their care plan continues smoothly after a hospital discharge. The nurse practitioners are not a substitute for a person’s primary care provider. While we collaborate with the health care team, it is important for individuals to continue seeing their primary care provider for routine medical concerns and chronic disease management. This ensures consistent and comprehensive care.
