Thunder Bay Regional Health Sciences Centre (TBRHSC) is committed to the identification, removal and prevention of accessibility barriers in compliance with standards that have been established under the Accessibility for Ontarians with Disabilities Act, 2005 (AODA). Accessibility refers to the design of products, devices, services, or environments for
people who experience disabilities.

TBRHSC invites you to consider the following questions and provide your feedback to help us address, remove and prevent barriers for persons with disabilities. We take feedback into serious consideration and especially welcome suggestions
for improvement. This survey is voluntary and confidential. Please do not identify yourself in your responses.
