What is Cardiovascular Rehabilitation?

Recovering from a cardiac or vascular event can feel overwhelming. You may be unsure of where to start and feel overloaded with information. The Cardiovascular Rehabilitation program is designed to guide and support you every step of the way.  Our goal is to help you improve your physical health, enhance your mental well-being, regain confidence in everyday life, and reduce cardiovascular risk factors.

The Cardiovascular Rehabilitation program helps patients in several different ways by providing education, guided exercise, and personal counselling. We focus on educating and motivating patients to understand their risk factors, be more active, eat a healthy diet, be smoke-free, and find alternative methods to manage stress. Our interdisciplinary team personalizes the program to best suit individuals’ goals and abilities. We want to help patients lead a healthy and independent lifestyle. We foster a Patient and Family Centred Care environment and encourage families and loved ones to attend initial appointments and counselling sessions. We offer this program in person, virtually, or a combination of both, to best suit individual needs across Northwestern Ontario.

Program Overview

Joining the Cardiovascular Rehabilitation program must be done by referral only from a specialist or your primary care provider (family physician or nurse practitioner). If you do not have a primary care provider, you may also be referred by a physician or nurse practitioner at a walk-in clinic, nursing station, or Emergency Department.

Please note that a referral is not a guarantee that you will be accepted into the program as we review each referral to ensure that the person meets the program’s criteria.

Your Journey through Cardiovascular Rehabilitation

Step 1: Initial Phone Call

Step 2: Complete a Six-Minute Walk Test or Regular Exercise Stress Test (If Required)

Step 3: Initial Appointments

Step 4: Group Orientation to the Cardiovascular Rehabilitation Gym

Step 5: Phone Call with Case Manager

Step 6: Exercise Program

Step 7: Education and Counselling Services
