Discharge Information

Please note that our discharge time is 11:00 a.m. for most patients. Please ensure that you have all of your belongings and health card, as well as aftercare instructions and prescriptions as needed. It is important that you understand all instructions about your medication, diet, activity, and return appointments. If you are unsure or have questions about anything, please ask your health care provider (physician or nurse) before you leave.

The team will assist in answering all non-acute services you require.

Once a discharge order is written you must make arrangements to leave the building to ensure the resources are available as soon as possible for patients requiring services.

Utilization Coordinators are available to assist patients who no longer require acute hospital care and are waiting for transfer to Rehabilitation, Long-Term Care, Chronic Complex Care, or Home Care. Utilization Coordinators are available from 8:00 a.m. – 4:00 p.m. Monday to Friday, excluding statutory holidays. Call Switchboard to page.
