Mapping A Stroke Survivor’s Journey in Northwestern Ontario

Join Esme and Keli, representing the Patient Journey Project Team, to walk you and your
family through the newly created stroke map as you navigate your stroke recovery journey
ahead. This session will involve a brief educational presentation with interactive group work
to help stimulate conversation and support. The journey after stroke is different for everyone,
whether your stroke was recent or years ago, lets learn from each other! Stroke resources
will be provided for all in attendance.
And of course, it wouldn’t be an afternoon without some light refreshments and great
conversation with each other afterwards.
