Please fax referrals to 1-855-249-4937, or referrals can be completed via the OCEAN platform (Search: Cardiovascular Services Central Intake through the Thunder Bay Regional Health Sciences Centre [TBRHSC] Cardiovascular Services pathway). If you have any questions about the status of the referral you have sent, please contact the Cardiovascular Rehabilitation front desk at 807-684-6780.
Physician or nurse practitioner referral is required for consideration of participation in the TBRHSC Cardiovascular Rehabilitation Program and regional satellite sites across Northwestern Ontario.
Please note: referral is not an automatic acceptance into the program.
Eligibility Criteria
Anyone who has had a previous cardiac event, has a cardiac diagnosis, has a vascular diagnosis, and three or more cardiovascular disease risk factors.
