Secondary Stroke Prevention was identified as a provincial priority by the eleven regional stroke networks in fall 2015. To address this priority, a working group with cross representation from the stroke networks was formed and the resulting deliverables of this work will be shared in the webinar along with a practical example and demonstration of using the SPC Toolkit.

Objectives: Upon completion, participants will be able to:
- Describe an overview of the deliverables of the Provincial Integrated Work Plan- Secondary Stroke Prevention (SPC)
- Provide an example of an SPC Self-Assessment & workplan developed using the SPC Toolkit to meet the Ontario Core Elements
- Demonstrate accessing resources within the online SPC Toolkit
