At Thunder Bay Regional Health Sciences Centre, Patient Advocates are dedicated to ensuring exceptional care by guiding clinical decisions with respect for patients’ and families’ preferences, needs, and values. They create a caring environment and collaborate with patients, families, and health care teams to enhance the overall patient experience and navigate the health care system.
Patient Advocates act as liaisons between patients, their families, and the health care team. They help resolve concerns, improve care, and ensure that patients’ voices are heard. Their responsibilities include:
- Meeting with patients and families to discuss concerns and needs
- Collaborating with health care teams to resolve issues
- Educating about rights and resources
- Assisting with care coordination and discharge planning
- Documenting interactions and follow-up actions
With a background in Nursing and Health Care Administration, Patient Advocates use their expertise to effectively communicate and address the complexities of patient care.
