Gloria is a Post Discharge Liaison within the Patient and Family Centred Care (PFCC) department at Thunder Bay Regional Health Science Center (TBRHSC). We recently caught up with Gloria to find out more about her role and what brought her to PFCC.

How did you get into patient and family centred care at TBRHSC?

My career journey began in the dental field, working as an assistant for six years. My initiative and love to help others grew. From there I transitioned into the health care field and became a Registered Practical Nurse in the summer of 2021. I worked full time on the 1A Oncology/Medical floor, gathering experience in acute care assessing and caring for patients.

Tell us about your role.

My role as a Post Discharge Liaison is to gather, interpret and assess data from discharge and support patients’ understanding of their diagnosis, treatment(s) and medication(s), in order to reduce re-admissions. I also act as a liaison to support the patient’s transition home from the Hospital. My day starts off researching all the discharged patients from the previous day, calling each patient and asking them a series of questions to identify any gaps in their transition home.

This directly reflects the PFCC core concepts as we listen and honor the patient and family experience; we communicate and share unbiased information regarding the patient’s diagnosis; and lastly, we collaborate with other departments within TBRHSC, community resources and facilities as well.
