How to obtain your vaccine receiptThe TBRHSC does not provide copies of vaccine receipts.

Vaccine receipts are available online through the Provincial Booking System at https://covid19.ontariohealth.ca/

Scroll to the bottom of the page. click “Continue” If you read, understand and accept the terms of use.
Complete the Identity Verification Information and click “Continue”. You will need your health card number.
Click “Continue”.in the yellow box titled “Vaccination Receipts”
Review your results and access a PDF receipt.
