As of May 21st, 2025, *ALL referrals for vascular assessment and service will now be received via the Cardiovascular Central Intake to be triaged to the RAVE clinic or the North of Superior Vascular Surgery offices.
*Excluding dialysis access-related, which continue to be managed via the Renal department of Thunder Bay Regional Health Sciences Centre (TBRHSC).
As always, referrals can be sent via “fax” to the Central Intake at 807-684-9561. Referrals can also be completed via the OCEAN platform through TBRHSC Cardiovascular Services pathway OR via the TBRHSC Surgical Services pathway.
Please use the link below to access the most up to date version of the Vascular Services Referral Form (FCS-490) which is to be completed for ALL vascular referrals and faxed (along with pertinent medical history, updated medication list and relevant diagnostic imaging if not available via Sectra PACS at TBRHSC) to 807-684-9561. Triaging will occur based on referral information received.
PLEASE NOTE: this is a NEW Vascular Services Referral Form as of April 2025. Please remove the discontinued RAVE Referrals (CS-1077) form from circulation within your office(s).
