The Rapid Access to Vascular Evaluation (RAVE) Clinic is accepting referrals for urgent non-healing wounds and any other urgent vascular concerns. Below is the RAVE referral form to be filled out by the referring Physician/Nurse Practitioner and faxed to the RAVE clinic at 807-684-9561.
