Presentation Objectives: At the end of this presentation you will be able to:
Describe the tools and resources developed by the 2017-18 PIWPs to support community and outpatient rehab, secondary stroke prevention and the knowledge translation, transfer, management and sustainability of past and present PIWP work; and
Consider how to utilize these tools and resources.
