Model of Practice

For Health Professionals

The Thunder Bay Regional Health Sciences Centre (TBRHSC) delivers integrated, interprofessional care through collaborative practice, guided by professional excellence, respect, collegiality, compassion, and accountability. Patients benefit from care that is well planned, holistic, consistent, and coordinated by a team of interdisciplinary healthcare providers.

As an academic health sciences centre, TBRHSC is committed to creating a learning environment that supports the development of current and future health care professionals. We welcome learners from Lakehead University, NOSM University, Confederation College and Oshki-Pimache-O-Win: The Wenjack Education Institute, and other academic institutions across Canada and around the world. Learners are immersed in a comprehensive, interprofessional education experience that extends beyond clinical knowledge and technical skills to include the values, behaviours, and relationships that underpin how care is delivered at TBRHSC. Through clinical practice, simulation, education, mentorship, and interprofessional collaboration, learners experience firsthand how patients and families are engaged as partners in care, and how health care professionals work together to achieve shared outcomes. In this way, our learners are not simply educated within our organization; they become immersed in our model of practice and contribute to a culture of inquiry, learning and continuous improvement. Our model of practice is grounded in Patient-and Family-Centred Care (PFCC), which provides the foundation for how we deliver care, engage with patients and families, collaborate as teams, and prepare the next generation of health professionals.

The TBRHSC has developed a uniquely integrated PFCC model that defines how care is delivered. It shapes how we design services, how we communicate at the bedside, and how we partner with patients and families in every aspect of care. With over 70 Patient and Family Advisors embedded across the organization, we move beyond consultation to true co-design; working alongside patients and families to shape policies, improve processes, and design care experiences from bedside to strategy. This approach drives measurable impact, strengthening safety, improving outcomes, and enhancing the overall care experience. We continue to advance PFCC through innovation in communication and system integration, ensuring care is shaped with patients and families, not for them. Click to learn more about PFCC at the Thunder Bay Regional Health Sciences Centre.
