Quality Improvement Plan
A Quality Improvement Plan (QIP) is a health care organization’s formal plan to its community. The focus of the plan is to drive improvements that address province-wide priority issues – advancing equity, inclusion, and diversity to achieve better outcomes and improve the care provided. Hospitals are legally required to submit a QIP to Ontario Health (formally called Health Quality Ontario) each year by April 1st. The Quality Improvement Plan is part of the Government of Ontario’s Excellent Care for All Act.
Overall, a QIP should be seen as a tool, providing a structured format and common language that focuses an organization on change. The QIP will drive change by formalizing a plan and facilitating shared dialogue to support continuous quality improvement processes.

How to Read the Quality Improvement Plan
Our 2024-2025 Quality Improvement Plan (QIP) is made up of three parts:
- Narrative – A document that provides a brief overview of our quality improvement plan, highlighting and listing our Hospital’s top priorities for the year.
- Work Plan – Information that includes our improvement targets and initiatives. The spreadsheet includes a core set of indicators and targets that align with system priorities to support quality care across the province.
- Progress report – Information on progress and improvement efforts from 2023-2024.
