The Surgical Intake General Surgery program accepts referrals for gallbladder and hernia only at this time. Once the complete referral is received, it is reviewed for accuracy and routed directly to one of the General Surgeons. Urology referrals for Surgical Intake are sent to a separate fax number. Please use Step 3 under the guidelines listed on the referrals as reference.
