Community and Long Term Care

Following a stroke, quality of life is the responsibility of many levels of service providers, from government and health system leaders to healthcare providers, educators, and health organizations. Healthcare and service providers in the community play a key role for support and direction for the stroke survivor and family. Following a stroke the person should have the opportunity to re-engage into their community and / or life in long-term care.

The NWORSN participates in initiatives within the community / long-term care sector to establish a coordinated and seamless system of transitions in our region that optimize quality of life.
