DESCRIPTION:The 5As Program: Improving obesity prevention and management in primary care teams.\nDenise Campbell-Scherer\, MD\, PhD\, CCFP\, FCFP\nUniversity of Alberta \nThursday\, March 29\, 2018\n12:00 p.m. – 1:00 p.m.\nPrimary care providers encounter patients living with obesity in their daily practice\, however\, there is a gap in empirical and theoretical understanding for the development of an effective and practical approach to supporting patients to improve health. The 5As Team Program (5AsT) has worked with interdisciplinary primary care teams as well as people living with obesity in order to: 1) change the quality and quantity of obesity management by creating resources and tools to support collaborative deliberation\, 2) understand what people want from their healthcare team\, and 3) understand the key elements\, impacts and outcomes of personalized consultations on everyday self-management.\nLearning Objectives:\n1. Recognize obesity is a chronic disease.\n2.
