Stroke Transitions Group
In alignment with Canadian Stroke Best Practice Recommendations, and in the context of health system transformation in Ontario, the overarching purpose of the Stroke Transitions Group is to work as a collective partnership to ensure that patients with stroke and their families experience smooth and efficient transitions between care settings, and are supported to successfully re-engage in the community.

NWORSN Community Stroke Prevention Clinic Nurses Committee
The NWORSN Community Stroke Prevention Clinic Nurses Committee advises and is accountable to the NWO Regional Stroke Network Steering Committee in the provision of stroke care and stroke services for secondary stroke prevention and with a smaller focus with primary stroke prevention as it relates to people at high risk for stroke, recurrent stroke, in Northwestern Ontario.
